Outcome of Transnasal Trasnssphenoidal Surgery for Pituitary Tumours

Abstract

INTRODUCTION: Surgical access to the sphenoid, sellar and para-sellar has been traditionally done by transcranial and transsphenoidal route. Transsphenoidal approaches can be done by transnasal and sub labial route. Transnasal Transsphenoidal procedure has been increasingly used in the resection of pituitary adenomas for its minimal invasiveness and less morbidity and mortality as compared with transcranial route.
OBJECTIVE: To determine the frequency of complications of transnasal transsphenoidal surgery for pituitary adenoma.
METHODOLOGY: This was Descriptive case series conducted at department of Neurosurgery Lady Reading Hospital, Peshawar from 1st February 2011 to 1st March 2013. A total 47 patients with pituitary adenoma fulfilling the inclusion criteria were included in this study. All patients were undergone transnasal transsphenoidal surgery. These patients were followed up for early complications of transsphenoidal surgery for one month. The data was analyzed by SPSS version 16. Frequency and percentage was calculated for categorical variables. Mean ± SD was calculated for age.
RESULTS: Out of 47 patients, there were 27 (57.4%) males and 20 (42.5%) females. The age of patients ranged from 20 to 70 years. In this study the overall mean age was 44.75 years. Majority of patients seventeen (36.17%) were in the age range of 41-50 years. Early post-operative complications were diabetes insipidus in five (10.6%) patients, CSF leak three (6.38%) patients, meningitis in one (2.12%) patient and epistaxis in one (2.12%) patient.
CONCLUSION: Surgical complications of Transnasal transsphenoidal surgery are minimal. Transnasal trans-sphenoidal surgery is a safe and effective treatment for pituitary tumour.
